Past medical history

Mr Bryant has type 2 diabetes well controlled by Metformin 500mg TDS. He has hypertension that is controlled with Ramipril 5mg OD and Bendroflumethiazide 2.5mg OD. He takes also Simvastatin 20mg and Aspirin 75mg OD.

He had a mild heart attack 10 years ago. He has not suffered any chest pain since then. He smoked 20 cigarettes per day for 40 years, giving up in his fifties. He has no breathing problems.
