There has just been a team meeting that you were unable to attend regarding a complicated discharge plan about a 58 year old woman who had a stroke 6 weeks ago. Mrs. Potts has been on the unit for 4 weeks now and has made significant gains. However, she is still having difficulty with judgement, problem solving and safety awareness.

Mrs. Potts is completely mobile and does not have any difficulties walking or with other aspects of physical functioning. Although her speech has improved, she continues to have difficulty "getting out the right words".
