(L) LE dominat Acces Direct/Direct Access—MD: Dr. Zarak (Family Doctor) Diagnostic/Diagnosis: (L) Hip Bursitis HPI: Patient reports having pain into the (L) hip for the past 4 years, but it has significantly gotten worse in the past 3 months. She was diagnosed with Hip OA 5 years ago, so she believes that the pain may be related. She is the main caretaker of her husband who has early onset Alzheimers, and helps with her Daughter in Law, who suffered a TBI 8 months ago. She is the main “driver” in the family and takes both her husband and daughter-in law to all their medical appointments. She believes that the consistent driving and walking to the appointments has been aggravating her hip. She saw her family doctor 2 weeks ago, and was offered a cortisone injection, but refused; instead, she asked to be referred to Physiotherapy. Antécédents/PMHx: (L) & (R) knee meniscal surgery 1995 (L), 1997 (R), Hypothyroidism, HBP (well controlled with medications), Arthritis ; hips, knees, fingers, and toes (bunions Bilateral) Imagerie and Tests: None Recent Traitements Antérieurs/Previous Treatment: Saw Physiotherapy following her Knee surgeries and found that it was very helpful.
