Further history

He has been resident at the Residential home for 18 months since the death of his wife. 

He has had to be assisted with his diet and fluids and has been taking in very little.

His urine has become highly concentrated and reduced in volume and frequency. It also has a distinctive strong smell. He has not had his bowels opened for 4 days.
