Your chief resident in pediatrics must examine the results of Amy Smith, an 8-year-old girl diagnosed with a second relapse of Acute Lymphoblastic Leukemia (ALL). Amy was initially diagnosed at 2 years of age. She relapsed one month after finishing the initial course, and had a further 18 months of therapy. Prognosis given at the last visit: “if she relapses again, only a bone marrow transplant might save her, and even then, the chances of success are very slim.”

Her mother is deceased. The father is traumatized and is waiting to inform the resident of the family’s final decision.
