VS scenario #4

Your patient is an 85 y.o. with hx of CABG 10y ago, stable in CHSLD. Comorbidities: hypertension, CAD, diabetes type 2 well controlled. Has nitro sublingual spray, but does not use according to staff, has not required it since admission 2 months ago. He is not on any maintenance programs because he is not interested in exercise, but has been slowly deteriorating and moving around less and less. You have been asked to assess his functional status and reassess his motivation for participating in maintenance programming.
